THIS APPLICATION IS FOR 12TH GRADE STUDENTS ENROLLED AT FAYETTEVILLE HIGH SCHOOL.
Applicants must complete all items in each section of the application and return with all attachments including:
1. Recent photograph
2. Applicants must provide High School Student ID#.
3. Two personal references used for Letters of Recommendation- list their name, address, phone number, relationship, and years known (at last one must be a high school teacher)
4. Letters of Recommendation can be attached to the application or sent to the Foundation directly from the reference (please email to firstname.lastname@example.org)
5. A writing sample for critique by the scholarship committee. You must submit different sample if you apply for more than one writing award.
