This is the application for New York-Presbyterian Hospital/Columbia University Medical Center's Clinical Informatics fellowship program for a position starting in July 2017. The program is accredited by the Accreditation Council for Graduate Medical Education (ACGME). The fellowship is housed within the Internal Medicine program but is open to all specialties. This is a two-year fellowship, and one slot opens each year. Prior to appointment in the program, each fellow must have completed an ACGME-accredited residency, a Royal College of Physicians and Surgeons of Canada (RCPSC), or College of Family Physicians of Canada (CFPC)-accredited residency program located in Canada.

DEADLINES: the deadline for submitting an application is Monday, October 31, 2016 at 8 PM EDT and the deadline for receipt of your letters of recommendation is Monday, October 17, 2016 at 7 PM EDT.
